Report Notification Contacts

Email contacts for notifying of a new or updated report available. Entries can be referrer, hospital (with optional specific department) or organisation (with optional specific department). Only locations can be registered with this form, referrers are self-registered through Resources / User functions but can be edited here.

Required fields are:

Display name

details of referrer or location, for display use only

Type

available options are 'hospital' or 'organisation' (referrer entries cannot change type)

Location/Identifier/Referral Source

for referrers this is their GMC number and will already have been entered at self-registration; for locations select from the suggestions, and for an organisation select the main hospital (if there is one)

Status

diagnosis status - new diagnosis only or all requests (new, relapse, follow-up and unknown status)

Contact address

valid email address - must be @nhs.net

Active

allows entry to be inactivated
