Fix teaching_cases.pl - requires additional variable after modification for NBT (9e7bbb5)

removed all BEGIN blocks from cron scripts - need to load Data::Dumper's Carp from local::lib before system Carp gets any chance of loading

M::Request::new_request() should not require auto_screen_config to save form_data - needed in HistoryAction regardless of auto_screen function; other minor bugfixes

new yearly register_and_error_log.pl

modified C::Patient::update_patient() to accept change of patient details for single records - NOT IMPLEMENTED YET - incomplete; patched error in M::Patient::update_patient() return hash; new teaching_cases cron; modified RXPB rules in activity_data cron
