LIMS::Local::QueryLog - fix permissions issue with previous commit (9171b6a & bf60ec8); format queries for console & log file using SQL::Abstract::Tree

Local::QueryLog * combine development queries (SELECT's, UPDATE's, etc) into single file * replace SELECT query log file names with simpler schema (development.sql, deployment.sql) * adapt log_rotate.pl script for changes to Local::QueryLog

removed all BEGIN blocks from cron scripts - need to load Data::Dumper's Carp from local::lib before system Carp gets any chance of loading

patched L::L::DBIxSimple to handle omiholders and placeholders in same query; new dbix-simple.t to test it; moved loop queries to data-maps in activity_data.pl; replace pipe-delimited file with xl file in gallium data for quintiles

print report function calculates composite immunohistochemistry p21/p53 result; replaced M::Request::get_previous_diagnosis_count() with get_previous_diagnoses() to supply data instead of just count, replacing redundant previous_diagnosis_data from C::Report::load(); printed reports now display 'amended report' details and highlights changed diagnosis; QueryLog creates separate logfiles for dev server user, to avoid lack of write permissions if files opened by Apache user & also to avoid polluting production logs with dev data

added auto-screen function to registration section

amended all cron scripts which used DateTime directly to use L::Local::Utils::time_now() instead, so time calculated correctly at day rollover (ie considers BST); patched Utils::time_now() to accept args for DT constructor (eg formatter)

added server_username to settings to allow Model to find username for logging application functions eg mail reports

bugfix to log_rotate.pl - need to check src file exists before creating IO object then calling unlink() on it

patched L:Dispatch to avoid error on undefined dispatch_path; moved L::Local::QueryLog dispatcher construction from _warn_handler() to BEGIN block to avoid re-creation per request; changed sql.log to Log::Dispatch::File::Rolling object; amended log_rotate.pl to process the new sql.log filenames; added new method ensure_warn_handler_capture() to L::Local::QueryLog for use with non-external fastcgi, where FCGI re-defines the warn handler set in L::L::Q so that sql statements go to Apache error log
