removed all BEGIN blocks from cron scripts - need to load Data::Dumper's Carp from local::lib before system Carp gets any chance of loading

commited changes to production server, made during report refactor on dev box - changed method of retrieving data for chart from screened_as to requested lab-tests; outreach CML patients use same chart as for Imatinib monitoring; activity_data.pl replaces default unknown dates< 1900 with NULL entry; bugfix to audit/revised_diagnosis.tt for users with 0 requests reported during selected duration

MAJOR CHANGE: combined send_message and send_attachment into a single method dispatch() in L::Local::Mail - consistently returns a Return::Value object so no longer handling returns from simple mail and attachments differerntly - affects app and crons; fix for resources meter chart for swap mem on Deb6 VM; patched M::Outreach::update_followup() to prevent unnecessary db lookups for non-duration followup (eg other), and prevented crash when attempting to update non-existant table entry; moved loading of local-lib above FindBin in hilis4.fcgi to avoid fastcgi error in Deb6 VM

patched C::Result::do_export_test_results() to only split $vars{category_id} if var supplied; added specimen type and result summary to export test results function; added specimen to unscreened.pl script

added specimen type breakdown info to user workload stats table; new unscreened requests cron
